Job : The Chronic Condition Nurse Educator (CCNE) integrates knowledge, skills and the principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education and management services for clients, families, the general public and health professionals. The CCNE uses a consultative approach, accepted standards and guidelines and current research findings to provide support to clients and health care professionals as appropriate. The CCNE is an integral part of the Primary Health Care Team. The employee works relatively independently and is expected to make decisions based on sound assessments in accordance with policies of Population and Public Health Services, the health region, the College of Registered Nurses of Saskatchewan (CRNS) Standards of Practice and other relevant legislation and standards. The CCNE develops relationships with, and is supported by, specialty programs that manage chronic conditions.
