Registered Nurse - Family Health Team (Toronto)

Position Summary

The Toronto Western Family Health Team delivers primary care services through a multi-disciplinary, inter‑skilled approach to diverse patient populations. The Registered Nurse (RN) will be an integral member of both the Family Health Team and the University Health Network Nursing group. In this role, the RN supports patients across all stages of life, through varying states of wellness and illness, while collaborating with patients, families and other care providers. Duties
