- Lead the Geriatric Outreach Team supported by Nurse Practitioners
- Manage inpatient on-call coverage at acute hospital sites
- Provide outpatient services across multiple geriatric clinics
- Develop tailored care plans for seniors aged 65+
- Oversee comprehensive geriatric assessments and care coordination
- Registration with the College of Physicians and Surgeons of Ontario
- Certification in Geriatric Medicine preferred
- Strong communication skills in English
- Commitment to academic involvement and teaching
- Experience caring for diverse patient populations
