This full-time temporary contract, ending April 30, 2027, offers you the chance to lead in primary care, particularly for those facing barriers. You will assess and treat patients, engage in case conferencing, and implement quality improvements within a collaborative environment. Experience needed includes working with high-risk clients and applying a trauma-informed approach. Key Responsibilities:
- Deliver comprehensive clinical services to patients
- Schedule and conduct home visits as needed
- Engage with clients using a trauma-informed lens
- Participate in case conferences and teamwork
- Develop and evaluate clinical protocols
- RN (EC) license to practice in Ontario
- Experience as a Nurse Practitioner in a primary care setting
- Skills in providing care to those facing health barriers
- Familiarity with trauma-informed care approaches
- Availability for evening shifts and an on-call rotation
