Remote Care Navigator for Senior Mental Health
Sailor Health
Sailor Health is hiring a Care Navigator to work remotely with Canada-based patients and families. You will guide older adults through the intake process with warmth, clarity, and professionalism, helping them connect with the right therapist and care team.
As the first emotional point of contact, you will support patients and caregivers, coordinate across care journeys, and help ensure a smooth entry into Sailor Health’s services.
