Resident Assessment Instrument (RAI) Coordinator

Location

The position is located at Seaton Village in North Pickering, Ontario. 

Hours of work are Monday to Friday day shift (7:00am - 3:00pm) with the occasional evening or night shift required for training purposes.

RAI Coordinator

Reporting to the Director of Care, the incumbent will be accountable for the day to day operations of the RAI-MDS assessment tool.

The incumbent will:

- Coordinate the RAI-MDS process within the home in accordance with MOLTC legislation, LHIN agreements, College Standards of Practice and CIHI requirements
- Prepare and maintain a schedule of RAI assessments for all disciplines and monitor regularly to ensure timely completion
- Collaborate with registered staff in identifying any significant health status changes which may warrant additional MDS assessments
- Provide instruction and training to direct care staff and the interprofessional team regarding RAI MDS, RAPS and care planning
- Act as subject matter experts within the homes in regards to clinical coding of the RAI instrument
- Coordinate and ensure completion of the annual AIS competency evaluations for all staff
- Compile and submit the quarterly electronic data submission to CIHI
- Review, analyze and trend quality indicators and share results and findings with the management team
- Run and analyze the RUG Weighted Patient Day Report quarterly and report current CMI financial funding levels to the Manager, Interprofessional Practice and Innovation and the home management team
- Analyze data reports and ensure all residents are classified in the appropriate funding levels, and adjust records as may be necessary to account for changes or increases in acuity, specialized medical services or clinically complex interventions
- Liaise with vendors, IT professionals and Regional staff regarding the development, installation, maintenance and education of clinical software systems used within the homes

The successful applicant will have:

- A diploma in Practical Nursing and a current General Certificate of Registration as a RPN with the College of Nurses of Ontario
- Completion of the six (6) core modules of the LTCF-RAI Coordinator/Educator/Back-up/Home Supervisor training
- A minimum of one year previous experience in a RAI coordinator role
- Sound knowledge of Canadian Institute of Health Information (CIHI) requirements, Local Health Integrated Network (LHIN) Accountability Agreements, Long Term Care Homes Act and Regulations
- Knowledge of Ontario College of Nurses Standards of Practice, MOLTC funding, quality indicators, RAI outcomes and RUG Groupings
- Familiarity of Personal Health Information and Privacy Act (PHIPA), Electronic health care record (eHCR) and Resident Quality Inspection protocols
- Strong clinical assessment skills, coupled with good judgment and problem-solving skills
- Demonstrated computer proficiency, along with excellent knowledge of clinical software systems
- Well-developed organization and time management abilities
- Demonstrated ability to lead, coach and mentor others
- A commitment to treating all clients and colleagues with dignity, respect and cultural sensitivity and a demonstrated willingness to adapt services to meet diverse needs

RAI Coordinator Salary Information

- The hourly rate range for this position is $39.72 to $40.82.

Conditions of Employment

All applicants are expected to comply with the Region of Durham's Code of Ethics and Code of Conduct Policies throughout the recruitment process. The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within six (6) months from date of hire. Additionally, tuberculosis screening will be required in accordance with the FLTC Act (O.Reg. 246/22). Proof of education, qualifications and any other job bona fide requirements will also be collected. Finally, compliance with the Mask Fit Testing Program is required and will be coordinated as part of the orientation process.
