Family Practitioner at Cowichan PCN

Join the Cowichan PCN team as a Family Practitioner. Collaborate with health professionals to provide comprehensive care across all age groups in a supportive clinic environment. The Cowichan PCN seeks Family Practitioners to enhance the current UPCC. In this role, you will work closely with Nurse Practitioners, Social Workers, and Medical Office Assistants to deliver personalized care from newborns to seniors. Key skills include diagnosing health conditions, managing chronic diseases, and providing mental health support. Key Responsibilities:

• Diagnose and manage acute and chronic health conditions
• Oversee chronic disease management for diverse conditions
• Provide women's health care services and mental health support
• Offer both in-person and telehealth appointment options
• Complete charting using Telus Med Access EMR

Requirements:

• Valid medical license to practice in British Columbia
• Experience with chronic disease and preventive care
• Skills in women's health and mental health services
• Commitment to collaborative patient care
• Willingness to work in a supportive clinic setting
