The ALC Care Teams Manager is responsible for the day-to-day supervision, coordination and support of the Care Team staff. This position plays a key role in ensuring the execution of care delivery, maintaining quality and consistency of care plans, and ensuring effective coordination across providers as well as contributing to program development, refinement of processes and continuous improvement efforts. The goal for this program is to oversee and provide non-medical wrap around supports with integration with health and mental services to successfully transition designated ALC patients back to their homes.

QUALIFICATIONS, SKILLS & EXPERIENCE REQUIRED

• Diploma/Degree in Social Work or related human services field, ACSW registration preferred
• Vehicle and valid Driver’s License
• Experience supervising or leading teams with strong leadership skills
• Minimum 3-5 years of experience in community-based social services or health-related programs
• Experience in case management and care coordination for complex clients
• Police Information Check including Vulnerable Sector is required
• Experience working with older adults and/or complex client populations

The starting salary range for this position is between $69,315 - $84,495 plus 100% Employer paid Health & Dental plan and 5% matched Pension plan . Salary considerations are based on assessment of the successful candidate’s education, skills, previous experience and other factors. Sage Seniors Association offers an outstanding work life balance with a 35-hour work week, 3 weeks of vacation to start and generous sick leave accruals . Sage also observes 13 statutory holidays in the calendar year. Sage is currently offering great flexibility with a hybrid working environment (remote Fridays), and the option for a compressed work week . Social Workers are re-imbursed for their annual ACSW registration fee as well as Social Worker Liability Insurance premiums.
