About the role:

What you'll do: 

• Client assessments
• Provide primary care in collaboration with other healthcare professionals  
• Medication administration
• Wound care
• Chronic Disease Management, e.g. diabetes care and education
• Specimen collection
• Rehabilitative programs
• Palliative Care
• And many other critical nursing skills 

What we're looking for: 

• Must be licensed as a Licensed Practical Nurse in the province of practice
• Community health experience is an asset
• Must enjoy driving, have a valid driver's license and access to a reliable vehicle
• A current and original copy of a satisfactory Vulnerable Sector Search and Child Abuse Registry Check required
• Must be able to wear Personal Protective Equipment (PPE)

Work conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics.
