ABOUT THE ROLE

Hogarth Riverview Manor is seeking compassionate and dedicated professionals to join our team at one of Ontario's largest long-term care homes. Our 480-bed facility is committed to people-centered care, respecting individual needs, cultural traditions, and lifestyles. Our staff works collaboratively with residents to promote independence and well-being, supporting them in achieving their highest level of self-care. Reporting to the Clinical Manager, the Registered Practical Nurse (RPN) is responsible for assessing residents, developing a plan of care, implementing treatments and evaluating the outcomes, in consultation with the resident/Substitute Decision Maker (SDM) and other health professionals, in accordance with the Standards of Practice for the College of Nurses of Ontario (CNO) and current Long-Term Care Legislation. The extent to which the RPN carries out these components of practice is determined by the complexity of the resident’s condition including the predictability of the outcome for the resident; the knowledge, skill and judgment required to assess for and deal with the risks involved; and the resources (human and otherwise) available in the environment for collaboration and/or assistance.

WHAT YOU BRING

Education/Experience:

• Current unrestricted registration with the Ontario College of Nurses (CNO)
• Practical Nursing Diploma from a recognized educational institute 
• Basic Life Support Certificate (BLS)
• Demonstrated experience and interest in working with senior populations, including providing end-of-life care
• Familiarity with the Long-Term Care Home Act and Regulations and other relevant legislation

Skills/Abilities:

• Excellent communication, leadership, critical reasoning and problem-solving skills
• Demonstrated understanding and commitment to people-centered care
• Computer proficiency, including Word and Windows, email communication and electronic documentation
• Excellent time management skills
• Recent experience in the administration of medications
• Ability to work independently and as part of a team
• Evidence of continuing professional development experience
• Demonstrated regular attendance at work

Assets

• Certification relevant to practice setting (Gerontology, Palliative Care, Dementia), including training in P.I.E.C.E.S. ™ and Gentle Persuasive Approaches is an asset
• Membership in a professional association (i.e. RPNAO) is an asset

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC)  where indicated, to ensure the absence of relevant criminal convictions.
