About the Role

We’re growing our Family Medicine team and recruiting Family Physicians to join our downtown clinics. This is a chance to build a long-term family practice with strong clinic support behind you, and to play a key role in growing longitudinal family medicine at this location as we work toward forming a new FHO+ model.

From day one, you will work closely with a multidisciplinary team to support patient-centered care. In the near term, the practice will focus on walk-in medicine as we build toward a FHO+ model, with a growing focus on longitudinal care as the model takes shape.

Locations: The Well and Harbourfront

Responsibilities

• Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence
• Support your patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity
• Apply your full scope to create thorough diagnoses and treatment plans which utilize our multidisciplinary team to support patients with all interconnected aspects of their health journey
• Engage with our multidisciplinary team and internal stakeholders through on-site education sessions and care innovation initiatives across HealthOne

Qualifications and Skills

• Patient focused and service minded approach to delivering excellent, comprehensive care
• Empathetic, collaborative approach and ability to work effectively with a wide range of internal and external stakeholders
• Commit to working at HealthOne for a minimum 4 days per week + 1 weekend day per month, with reliable scheduling
• Licensed to practice as a Family Physician in Ontario and in good standing with the CPSO
• 1+ year of experience preferred, however new grads are also welcomed
