ABOUT THE ROLE

As a Registered Practical Nurse with our High Support Housing Program , you’ll play a key role in helping clients reach their highest level of independence in a supportive, home-like environment. Our program provides 24-hour on-site clinical support within accessible residences, fostering collaboration between Nursing Staff, Community Support Assistants, and Social Work. Here, you’ll build meaningful relationships, empower clients in their daily living, and contribute to a dynamic team that values compassion, inclusion, and recovery-focused care.

WHAT YOU BRING
Education/Experience:
- Current unrestricted registration with the Ontario College of Nurses (CNO)
- Practical Nursing Diploma from a recognized educational institute
- Scope of Practice must include physical assessment and pharmacology
- Basic Life Support Certificate (BLS) and First Aid Certification
- Experience supporting individuals living with serious mental illness, intellectual disabilities, and/or concurrent addiction challenges
Skills/Abilities:
- Strong skills and knowledge in community assessment, psychosocial rehabilitation, crisis intervention, treatment, and recovery support
- Knowledge of psychiatric illnesses, their diagnosis and treatment, and principles of Psychosocial Rehabilitation (PSR) and recovery-oriented care
Completion of Level 1 Food Safety training or equivalent
Strong reasoning, problem-solving, decision-making, and workload management skills
Effective communication skills (verbal, non-verbal, and written) with strong interpersonal abilities
- Demonstrated understanding and commitment to people-centered care
- Computer proficiency, including Word and Windows, email communication and electronic documentation
- Excellent time management skills
- Ability to work independently and as part of a team
- Demonstrated regular attendance at work
Assets
- Candidates with past community experience in providing personal and clinical support to clients an asset
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Valid Ontario driver's license: Class G
