Nurse Case Manager at Cencora

Join Cencora as a Nurse Case Manager on a 6-month contract, supporting patients and healthcare providers with expert guidance. Leverage your nursing skills to enhance patient care and experience. This role focuses on facilitating exceptional patient support services and improving pharmaceutical commercialization. As a Nurse Case Manager, you will provide top-tier assistance, manage case protocols, and ensure effective communication among patients, families, and healthcare teams. Your contributions will directly impact healthcare outcomes and build relationships across Cencora’s services. Key Responsibilities:

Support patients with medication access and inquiries
Document patient interactions within the electronic health record
Collaborate with healthcare providers for therapy logistics
Train new Nurse Case Managers on processes
Attend relevant meetings and conferences for knowledge sharing

Requirements:

Licensed Nurse (RN or RPN/LPN) with active registration
2 to 5 years in healthcare or pharmaceutical settings
French and/or English fluency as required
Experience in case management and patient support
Strong analytical and problem-solving skills
