The Position
The Social Worker contributes to the qathet Complex Care Housing (CCH) program—a partnership between Lift Community Services, Vancouver Coastal Health, and Tla’amin Nation—by providing psychosocial assessment, counselling, and coordination of supports for individuals with complex mental health, substance use, and social challenges. The role assists clients and families to strengthen coping capacity, access community resources, and achieve stability through person-centered and trauma-informed practice within a collaborative interdisciplinary care model.

Qualifications
- Bachelor’s or Master’s Degree in Social Work from an accredited program.
- Current full registration with the British
- Columbia College of Social Workers (BCCSW).
- Minimum three (3) years of recent related experience working with individuals experiencing complex mental health, substance use, and social challenges, or an equivalent combination of education, training, and experience.
- Demonstrated ability to provide trauma-informed, harm reduction, and culturally safe care.
- Strong interpersonal, counselling, and advocacy skills.
- Ability to work collaboratively within a multidisciplinary team.
- Proficiency in clinical documentation and use of electronic records.
- Valid BC Driver’s License and access to a reliable vehicle.
- Criminal Record and Vulnerable Sector Check clearance..

Working Condition and Physical Requirements
- Based within housing and community health settings, including in-office and outreach environments.
- May work with individuals experiencing complex trauma, substance use, or behavioural challenges; requires calm and compassionate responses.
- Involves moderate physical activity including walking, sitting, and occasional lifting up to 20 lbs.
