Fraser Health is looking for a Social Worker with a Bachelor’s degree and two years of clinical experience. In this position, you will engage clients through high-risk screening, provide psychosocial assessments, and develop comprehensive care plans alongside interdisciplinary teams. Key Responsibilities:
• Conduct social and psychosocial assessments for clients
• Provide therapeutic counseling services to clients and families
• Facilitate client autonomy in recovery planning
• Monitor psychosocial situations and revise care plans
• Educate healthcare providers on psychosocial aspects
Requirements:
• Bachelor's degree in Social Work from an accredited institution
• Two years related clinical social work experience
• Full registration with the BC College of Social Workers
• Valid BC Driver's License
• Strong communication skills for effective collaboration
