We are looking for a full-time Community Support Worker to join our team!

 As a Community support worker, you will participate as a member of a multi-disciplinary team to provide crisis intervention treatment, rehabilitation, and support services to persons with severe and persistent mental illnesses allowing them to live successfully in the community by demonstrating best practice competencies in psychosocial rehabilitation.

Status: Full-Time permanent, 37.5 hours weekly, including some evenings and weekends. Note, this posting is for an active vacancy.

Base Salary:  $62,425.03 - $73,441.21 annually (Candidate qualifications will be considered when determining the starting salary)

Location:   This position is based out of our office at 60 West drive, Brampton, Ontario. Due to the nature of the program, this role is on-site and community based.

What you will do

• Provide support services for a caseload of clients as their primary worker within the team.


• Establish a supportive, ongoing relationship with clients and their support systems


• Develop a Crisis Plan with each client and their supports to support and maintain client wellness.


• Establish and monitor a recovery plan based on clients’ goals


• Provide on-going support, life skills coaching, pre-employment coaching and appropriate referrals to enable clients to achieve their goals


• Monitor clients’ mental health and initiate intervention to promote wellness


• Provide education on mental health and wellness to clients and their families and


• other supports


• Build positive relationships with appropriate informal supports as identified by clients


• Assist in vocational interventions by providing individual vocational-supportive counseling to establish vocational or career goals


• Provide practical help, supports and advocacy to help clients obtain the necessities of daily living including medical and dental health care, legal and advocacy services, financial support, supported housing, money-management services, and transportation


• Work effectively and collaboratively with the Assertive Community Treatment Team and other program teams, internal and external to CMHA Peel Dufferin

What you need to join our team

• University degree in social work (BSW) or related discipline


• At least 3-5 years of experience with individuals with serious and persistent mental illness, outpatient in the community


• Good family counseling skills and experience with dysfunctional family systems


• Basic understanding of psychotropic medications


• Excellent oral and written communication, organizational and problem-solving skills


• Experience with OCANs and client records documentation


• Ability to work with the diverse client group, considering issues of equity and from an LGBQT positive framework
  


• Current CPR/First Aid certification


• Valid Canadian driver’s license and access to an insured automobile in good repair 


• A satisfactory Vulnerable Sector Screening (Police Check) report
