Program Overview – Community Care Unit, Office of the Vice President
WoodGreen provides a range of programs from outreach case management services to comprehensive housing and stabilization services to support individuals who have experienced or are at risk of homelessness to help them live meaningful lives.

The Senior Director, Supportive Housing Programs and Community Transitions is responsible for the effective operation of the support programs through initiating annual program planning and evaluation; staff recruiting, supervision, training and performance reviews; compliance to relevant legislation and organizational policies; program outreach; protocol development and advocacy; representing the activities of the program within the community, the organization, coalitions and partnerships, and other community services.  This position is expected to coordinate and provide support services in accordance with best practices in community mental health care, harm reduction principles, organizational policies, Ministry of Health (MOH) and Ontario Health requirements, Residential Tenancies Act (RTA), City of Toronto, health related authorities, and all other relevant legislation.

The Senior Director will develop and promote shared goals, accountabilities, workflows, and quality improvement processes aligned with the missions, strategic directions, and accountability frameworks of both Michael Garron Hospital and WoodGreen Community Services. Through strong partnership, cross-sector collaboration, and systems leadership, this role will help define and implement a social medicine and supportive housing strategy for some of the most complex patient populations in East Toronto. 

  Role Overview
The Senior Director, Supportive Housing and Community Transitions, is a new partnership role between Michael Garron Hospital and WoodGreen Community Services designed to advance Supportive Housing, and integrated health and social care for East Toronto. Working under the joint leadership of senior leaders at both organizations, the Senior Director will bridge hospital and community services for patients and clients with highly complex health, mental health, addictions, housing, social, and functional needs.

This position is accountable to the Vice President, Community Care Unit, and is responsible for the Supportive Housing Programs within WoodGreen Community Services as well as reporting to the Executive Vice President, Clinical Programs and Quality at Michael Garron Hospital.  This role is responsible for operational oversight of the Supportive Housing Programs at WoodGreen Community Services while also providing strategic and operational leadership to support safe, coordinated transitions from hospital to supportive housing and community-based services, with a focus on improving outcomes for people who experience persistent barriers to discharge, housing stability, and ongoing care.

Specific Responsibilities
Strategic Leadership and Program Development

• Provides vision and direction for the portfolio in alignment with organizational mission, vision, and strategic priorities of both WoodGreen and MGH.
• Supports strategic planning and develop annual operational plans that support organizational priorities.
• Leads strategic planning to develop a shared social medicine and supportive housing strategy for WoodGreen and MGH and incorporate new models of care into WoodGreen’s Supportive Housing programs.
• Consolidates best practices, feedback from stakeholders and in partnership with clients and the community to design and continuously improves programs that address emerging community needs.
• Liaises with partners, government, OHTs to ensure the effective coordination of programs and supports improved system integrations to achieve enhanced client outcomes
• Ensuring strategic growth through forming of new partnerships or expanding current programs and procurement of government grants and contracts.
• Builds on best practices and feedback from stakeholders to identify options and opportunities to address emerging community needs.

  Operations Oversight and Service Delivery

• Responsible for overall operations and quality assurance and initiatives of designated portfolio of programs and services, including the development of operating plans and outcomes, special projects and new initiatives.
• Effectively oversees all programs in SH demonstrating consistent quality of financial oversight, program monitoring and evaluation and quality improvement, communications, allotment of time and resources to achieve operational and organizational strategic goals
• Works with SH leadership and staff to develop processes that ensure delivery of consistent, high quality wraparound services—housing stability, case management, health supports, life skills, etc.
• Ensures the day-to-day operations of the SH programs are compliant with any legislation, standards, or governing and funder agreements.
• Oversees budgetary planning, monitoring and reporting for the designated portfolio of programs
• Ensures the effective utilizations of staff, volunteers, and students throughout the programs while also ensuring a platform of support, training and appropriate professional development.
• Identify and mitigate risks related to safety, property, tenancy, hoarding, eviction, etc.
• Ensures the anti‑racism objectives and the community development principles of the organization are achieved within the units.
• Supports development of, and ensures that all program protocols, policies, safety procedures, and evaluation frameworks are in line with organizational standards, relevant legislation, Professional regulatory College requirements, Accreditation and Compliance standards.

Knowledge Management

• Develops the necessary systems, processes and tools to better support the facilitation, collection and sharing of data and knowledge that is generated by Programs.
• Ensures that key project outcomes, policy, advocacy and legislation are evaluated and leveraged for maximum community and organisational impact.
• Leads evaluation and quality improvement initiatives within the portfolio.
• Supports knowledge translation for WoodGreen and MGH’s social medicine and supportive housing strategy across East Toronto.

External Relationships

• Represents WoodGreen and MGH at several tables on matters relating to systems planning and implementation, coordination and funding related to Supportive Housing and supports for individuals at risk or experiencing chronic homelessness.
• Builds and strengthens internal partnerships through purposeful collaboration, coordination and integration across other WoodGreen and MGH departments
• Builds, strengthens, and grows external partnerships, including government, funders, and nonprofit networks and organizations.

What You Bring to the Team

• Master’s degree in Social Work, Public Administration, Housing or related field (or equivalent experience).
• Minimum ten (10) years recent relevant management experience in related field required
• Experience managing unionized teams is an asset.
• Proven leadership in planning, development and evaluation.
