Licensed Practical Nurse

As a Home Care Licensed Practical Nurse, you will report to the Home Care Manager and will be responsible to assess, plan, monitor and evaluate clients in consultation with the Registered Nurse Case Manager. You will be responsible for coordinating and providing quality care to clients in the community and will provide direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Required Qualifications

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications

Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).

• Completion of an accredited Licensed Practical Nurse education program.
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Experience in Home Parental Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching.
• Minimum two years’ experience within the last five years’ of Home Care experience
• Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes Effective oral and written communication. Working independently and as member of a team. Working with people in a health care environment with professionalism and respect. Positive approach and good listening skills in dealing with challenging situations with empathy and sincerity. Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift. Physical exertion including bending/walking/sitting or standing for long periods of time. Keyboarding/moussing documentations for prolonged periods of time. Ability to perform all duties in accordance with related legislation. Preferred Qualifications: Knowledge of the EPIC-Connect care system
