Care Access Representative Toronto

Coordinate home care scheduling at CBI Home Health in Toronto, using communication, problem-solving, and scheduling software in a fully on-site role.

As a Care Access Representative, you will manage direct-care staff scheduling within a high-volume contact center. You will handle calls, chats, data entry, and intake while supporting clients, families, and care providers. Your work helps maintain continuity of care.

Key Responsibilities:

• Assign care providers using scheduling software accurately
• Handle inbound and outbound calls and chats
• Update client records, care plans, and databases
• Address scheduling issues and escalate when necessary
• Maintain relationships with clients, families, and providers

Requirements:

• Minimum Grade 12 education
• At least two years’ customer service, scheduling, or coordination experience
• Microsoft Office and scheduling software proficiency
• Keyboarding speed of 35–45 words per minute
• Availability for rotating shifts Monday through Sunday
