Occupational Therapist

SE Health is currently hiring Occupational Therapist to work within the Rehab Services portfolio in their Community Stroke Rehab clinic ( (955 Major Mac) in Vaughan . The Occupational Therapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients.  The successful candidate will be familiar working in a team environment and in delegating treatment to appropriately qualified therapy assistants and be available to potentially work early evening hours as needed.

As the successful candidate, you are a graduate of an accredited program in Occupational Therapy and are a member in good standing of the College of Occupational Therapists of Ontario. You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.

Responsibilities

• Conducting assessments
• Formulating goals and recommendations
• Planning and implementing client-centered, goal-directed treatment programs & working with Therapy Assistants
• Assist in the promotion of occupational therapy services to external referral sources
• Participating actively in the planning, implementation and evaluation of the program
• Provide ad-hoc home based OT therapy as needed

Requirements

• A graduate of an accredited program in Occupational Therapy and are a member in good standing of the College of Occupational Therapists of Ontario
• Recent experience in managing patients in acute/rehab programs is preferred
• Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team
• 2 years of clinical experience in Stroke/Neurological patient populations is preferred
• Experience in assessment and generating treatment plans for patients with stroke/neurological disorders
• Knowledge of stroke best practices
• Experience with Chronic disease management is a good asset
• Sound knowledge of community resources is an asset
• NDT training is an asset
• Effective interpersonal, organizational and communication skills
• Strong customer service focus and excellent problem-solving skills required
• A positive work and attendance record is essential
• Ability to adapt to new computer applications
• CPR Level C + AED, and Standard/ Intermediate First Aide certificate
• Valid G license required
• Criminal background check, Vulnerable sector check and Immunization record (current)
