Flemingdon Health Centre
Flemingdon Health Centre in North York, ON, is seeking a Chronic Disease & Mental Health Service Worker for a 6-month contract, part-time 0.6 FTE (21 hours/week) with possible evenings and weekends. The role reports to the Manager of Chronic Disease and Mental Health and involves multidisciplinary collaboration to support community health needs.
