Expected Start Date: August 11, 2026 Union: SUN Facility: Grenfell Health Centre City/Town: Grenfell Department: Primary Care Sites Type: Full-time temporary Expected Up to Date: December 31, 2026 FTE: 1 Shift Information: Days Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: Yes Job : The Chronic Condition Nurse Educator integrates knowledge, skills and the principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education and management services for clients, families, the general public and health professionals. The Chronic Condition Nurse Educator uses a consultative approach, accepted standard and guidelines and current research findings to provide support to clients and health care professionals as appropriate. Human Resources Exemption: No Education- Bachelor of Science in Nursing (BScN)
- Degree in Nursing with courses in supervision or degree in edu./adult learning
Licenses
- Basic Life Support Certificate (BLS)
- Basic Life Support Instructor Certificate (BLS)
Competencies
- Verbal communication skills
- Computer skills
- Written Communication Skills
Experiences
- Experience in teaching adults, facilitation skills, and leading group activities
- 3 years exp chronic disease/complex illness edu. or chronic disease mgmt. required
Other Education & Training
- Nephrology Certification is desired
- Basic Cardiac Life Support Certificate (BCLS)

- Knowledge of program planning process
- Skills in program planning, consultation and evaluation
