The Registered Nurse (RN) is responsible for the provision and management of cost‑effective nursing care to individuals and families in the hospice setting. Care is delivered within Carefor’s quality and risk management framework and in accordance with the standards and scope of practice established by the Ontario College of Nurses. The RN provides clinical leadership, teaches and delegates tasks to regulated and unregulated care providers, and ensures high‑quality, client‑centred care.

- Provide complete and relevant assessments of clients and families using established procedures.
- Develop individualized care plans in collaboration with clients and families.
- Incorporate discharge planning into the care plan.
- Implement nursing interventions according to the established care plan.
- Perform technical nursing skills safely and in accordance with nursing principles and Carefor policies.
- Evaluate and document client and family responses to care using established documentation tools.
- Utilize effective interviewing and communication skills to obtain client information.
- Clearly and accurately document nursing processes according to Carefor standards.
- Revise care plans based on client outcomes and changing needs.
- Demonstrate negotiation skills and respect for client choices and decisions.
- Provide health teaching using adult learning principles and established materials.
- Integrate current research and best practices into clinical care.
- Communicate client status regularly with the interdisciplinary team.
- Refer clients appropriately to other nursing providers and disciplines.
- Document rationale for referrals, changes in visit frequency, and discharge decisions.
- Provide cost‑effective, organized, outcome‑focused care.
- Demonstrate effective time management and planning.
- Delegate care appropriately to family members, RPNs, and other providers.
- Promote client‑directed care, self‑care, and independence.
- Act as a clinical resource and mentor for staff and students.
- Support clients and families in making informed decisions about health and service needs.
- Act as Team Leader in the absence of the Hospice Manager.
- Practice in accordance with Carefor’s quality and risk management framework, including infection control and client safety.
- Demonstrate knowledge of WHMIS, universal precautions, and safe work practices.
- Report equipment defects, hazards, and health and safety concerns promptly.
- Ensure all workplace incidents are reported according to legislation and Carefor procedures.
- Participate in decision‑making and continuous quality improvement initiatives.
- Maintain competence through ongoing education, including CPR, high‑tech training, and palliative care initiatives.
- Collaborate with management and colleagues in evaluating personal practice (e.g., CNO reflective practice).
- Function in accordance with Carefor policies, procedures, strategic directions, and applicable legislation.
- Certificate of Competence/License as a Registered Nurse in Ontario.

- Certificate in Palliative Care (preferred).
- Clinical practice experience (mandatory).
- Experience in Palliative Care (mandatory).
- Experience in Community Health (preferred).
- Strong assessment, risk identification, and risk management skills.
- Skills in collaboration, consultation, and counselling.
- Enhanced chronic disease monitoring and management skills.
- Strong health teaching abilities.
- Excellent verbal and written communication skills.
- Strong interpersonal skills and ability to lead a multidisciplinary team.
- Ability to work effectively with regulated and unregulated care providers and volunteers.
- Strong commitment to and empathy for palliative clients, with an understanding of the physical, psychosocial, social, and spiritual aspects of end‑of‑life care.
