The Occupational Therapist is a member of the interprofessional Dual Diagnosis Consultation Outreach Team (DDCOT) serving Southeastern Ontario. The team provides assessment, consultation, and short-term treatment for people over the age of 16 with a dual diagnosis (an intellectual disability and a diagnosed or suspected mental illness including behavioural disorders) . Utilizing a shared care model, the team works in collaboration with community partners and family physicians to reduce the impact of mental illness and improve the quality of life for adults with a dual diagnosis and their care providers. As a member of the team, the Occupational Therapist will utilize discipline specific knowledge , skills and abilities in order to enhance and develop resources and will provide to service to clients throughout South Eastern Ontario , based from the Kingston  office . 

Education and Experience:

• Master’s Degree in Occupational Therapy, preferred
• Current, unrestricted registration with the College of Occupational Therapists of  Ontario 
• Minimum of three years of experience in dual diagnosis/psychiatry or similar equivalent, preferred
• Clinical experience working with individuals with a dual diagnosis, preferred
• Experience working in a wide range of community settings including clients’ homes
• Proven experience working with families and community agencies
• Community development and experience preferred
• Experience providing case management services

Position Duties & Responsibilities:

• Provides comprehensive assessment, consultation, and treatment for a clinical caseload, along with follow-up and discharge planning once other interventions are complete
• Reviews and evaluates the treatment and ongoing care plan, in consultation with the client/caregiver/care provider
• Provides occupational performance problem identification and occupation performance analysis to determine outcomes for OT intervention , recommending interventions such as remediation of impairments, developing strategies for adaptation, facilitating skill development, and strengthening individual’s resources, matching the individual’s strengths, values and activity .  ( e.g. functional, environmental, cognitive and physical assessments, evaluation of daily use of time) 
• Identifies  strengths and skills  of individuals  in achieving  occupational goals,  engagement and community participation
• Assists  individuals to  plan and  structure social  and leisure  time activities  on evenings, weekends and holidays through the use of natural supports and community resources
• Identif ies on a case by case basis , the educational needs of both caregivers / providers and provide s the education in the most appropriate format
