Duties

• Conduct thorough patient assessments and develop individualized care plans.
• Provide medical diagnoses and prescribe appropriate treatments in accordance with state licensure.
• Manage case coordination for patients with complex health needs.
• Collaborate with multidisciplinary teams utilizing electronic health records systems such as Accuro.
• Offer patient education on health maintenance, medication use, and disease prevention strategies.
• Assist in managing laboratory tests and interpreting results to inform clinical decisions.
• Document all patient interactions accurately in medical records while adhering to confidentiality standards.

Skills

• Solid knowledge of medical terminology, medication administration, and case management principles.
• Experience with electronic health record systems such as Accuro is preferred.
• Excellent patient care skills with a compassionate approach to diverse patient needs.
• Ability to perform phlebotomy and manage dementia or other complex care cases effectively.
• Skilled in pain management strategies and laboratory procedures relevant to primary care practice.
• Effective communicator capable of collaborating within multidisciplinary healthcare teams in medical offices or community settings.
