Nurse Case Manager

Remote nursing case manager position supporting patient and provider services for pharmaceutical commercialization in Canada. Role involves coordinating patient access to therapies, liaising between patients and healthcare providers, and managing case documentation and adherence monitoring.
Responsibilities
- Provide patient and caregiver support regarding disease information, therapies, and medication access based on manufacturer materials
- Conduct patient assessments and serve as liaison between patients, families, and healthcare providers
- Document patient interactions in electronic records and monitor patient therapy adherence and disruptions
- Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternative options
- Coordinate treatment logistics with healthcare providers, pharmacies, and clinics; report adverse events following program procedures
Requirements
- Licensed Nurse (RN or RPN/LPN) with active provincial registration
- Diploma in healthcare or related field; bachelor's degree preferred
- Strong problem-solving and analytical skills
- Ability to work independently and collaboratively in a team environment
- Excellent communication and interpersonal abilities
- Experience with patient assessment and case management protocols
Compensation: $43,800 - $68,640 CAD
