JOB SUMMARY

This position works in conjunction with St. Joseph’s Lifecare Centre staff, contract companies, and community networks, to maintain resident independence and promote the optimal mental, physical, social, intellectual and spiritual well-being of the resident. Restorative Care will work in accordance with all policies and procedures of the Lifecare Centre. As is required of all employees, Restorative Care will work in a safe and healthy manner and follow the regulations outlined in the Occupational Health and Safety Act.

KEY ACCOUNTABILITIES & SUCCESS CRITERIA

• As part of the Nursing Rehabilitation program, Restorative Care completes assessments, planning, implementation, and evaluation of residents for an individualized program for activities of daily living. All program information is provided to the RAI/MDS Coordinator.

• Provides education to staff as needed i.e. ROHO cushions, lift and transfer techniques

• Using the computer regularly (Word, Excel, ActivityPro, Point Click Care) to input resident data to maintain current files.

• Completing and providing month-end attendance and statistics for therapies

• Assessing and conducting appropriate related therapeutic and restorative programs.

• Participating in the assessment of residents recommended for an internal transfer.

• Conferring and liaising with program staff, family/POA, OT, PT, SLP and other health team members individually and in conference, to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating treatment programs.

• Observing residents during treatments, compiling, and evaluating data on residents’ responses to treatments and progress while working in conjunction with PT to create most appropriate program for each resident.

• Support annual program evaluations for required programs including: PASDs, Restraints, Falls and Restorative.

• Providing in-services when required in related fields.

• Attend all post admission and annual conferences.

• Participating in pertinent committees including Falls, PASDs/Restraints and Skin and Wound.

• In conjunction with the Physiotherapist conduct all transfer assessments for residents.

• Complete all assessments, audits and appropriate documentation for all the following: mobility equipment and devices, resident information frames, bed entrapment, PASDs/Restraints, falls, alarm devices, therapeutic surfaces, balance inter RAI-MDS assessment.

• Falls team member – Conduct daily fall audit, coordinate falls huddles with all interdisciplinary team members, management of red leaf program for high-risk fallers.

• Liaison between resident, family, Physiotherapist, and preferred vendor for all new adaptive device assessments.

• Organizing and maintaining the safe working order of adaptive and mobility equipment as required, either daily or through weekly clinics with local vendor.

• Management of all adaptive and mobility equipment including but not limited to wheelchairs, cushions, walkers, recliner chairs, transfer discs, transfer poles, adaptive eating utensils, alarms, transfer slings and fall mats.

• Instructing, motivating, and assisting residents to learn and improve functional activities such as pre-ambulation, transfer, ambulation, and ADLs.

This position may be required to perform additional related job functions beyond this description.

QUALIFICATIONS & SKILLS

• Post-secondary degree or diploma in a related field

• OTA/PTA certification an asset

• Restorative Care Certificate (required)

• GPA an asset (Gentle Persuasive Approach)

• Experience in related roles (quality improvement, accreditation) an asset.
