Intervenant Bilingue en Réhabilitation

Hôpital Montfort

Devenez intervenant dans le programme de santé mentale de l'Hôpital Montfort à Ottawa. Ce rôle nécessite une évaluation psychosociale et l'élaboration de plans de traitement individuels. Sous la direction du Gestionnaire clinique, vous intégrerez une équipe communautaire bilingue offrant des services intensifs en santé mentale. Vous serez chargé de préparer des plans de traitement en collaboration avec l'équipe interdisciplinaire. Votre mission est de favoriser le bien-être des patients souffrant de maladies mentales persistantes dans leur milieu de vie. Key Responsibilities:

Évaluer les besoins psychosociaux des patients
Élaborer des plans de traitement individualisés
Assurer le suivi auprès des patients en crise
Collaborer avec des intervenants externes
Enseigner aux patients et à leurs familles

Requirements:

DEC en sciences de la santé ou domaine pertinent
Certification en Cardio-secours reconnue
3 ans d’expérience en intervention de crise
Connaissances en réhabilitation psychosociale
Bilingue français et anglais à niveau intermédiaire
