Join CareRx as a Clinical Consultant Pharmacist in the Greater Toronto Area, focusing on enhancing senior care through specialized pharmacy services. Collaborate with healthcare teams to improve resident outcomes in long-term care communities. In this full-time role, you'll engage with onsite physicians and nursing staff to maintain continuity of care at retirement and long-term care homes. Your expertise will drive clinical pharmacy activities aligned with CareRx policies and health service standards. Relationship-building with community leadership is essential for ensuring optimal pharmacy services. Key Responsibilities:
- Act as liaison between CareRx and care homes
- Travel regularly to various LTC and retirement communities
- Conduct thorough medication reviews for residents
- Complete required documentation and billing processes
- Provide educational sessions on health topics
- Insured and licensed with Ontario College of Pharmacists
- Experience in healthcare collaboration and customer service
- Strong planning, organization, and communication skills
- Familiarity with Kroll Windows and Microsoft Office
- Valid driver’s license for community travel
