Community Nurse Role at SE Health

Responsibilities encompass conducting assessments and developing care plans based on clients' unique needs. This position supports professional growth while ensuring quality client care. Key Responsibilities:

Execute comprehensive assessments for each client
Develop tailored care plans based on assessments
Act as the primary point of contact for clients
Facilitate connections to community support services
Assist with care transitions for clients

Requirements:

Certificate of Competence from the College of Nurses of Ontario
Flexibility for various shifts, including weekends
Clear criminal background and updated immunizations
Must obtain SE Health required certifications
Current CPR certification required

Contribute to quality health services and teamwork as an essential nurse at SE Health. #J-18808-Ljbffr
