Cognitive Support RPN at Baycrest

Join Baycrest Hospital as a Registered Practical Nurse in the Cognitive Support Unit. Provide exceptional care for older adults with cognitive impairments in a permanent part-time role. The RPN will work within a specialized cognitive support team, delivering comprehensive, client-centered health services and assessments. Collaborating closely with Registered Nurses, this role focuses on developing individualized care plans tailored to the unique needs of residents. Your expertise will help enhance the well-being of seniors facing cognitive challenges. Key Responsibilities:

Provide clinical expertise in collaboration with RN
Plan and evaluate nursing interventions and treatments
Deliver client-centered care with the interdisciplinary team
Conduct thorough client assessments
Collaborate on individualized care plan development

Requirements:

Active registration with College of Nurses of Ontario
Current BCLS Level C certificate required
Long-Term Care experience preferred
Psychogeriatric experience is an asset
Certificate in Dementia Care preferred
