CBI Home Health is looking to hire an After Hours Integrated Care Lead (RN) for Newmarket / Bradford region - 12 Months Contract

Salary Range: The salary range for this position is $75,000 – $90,000 yearly .

What you will need to be successful:

• Registered Nurse in good standing with the College of Nurses of Ontario (CNO)


• 3 - 5 years of supervisory experience


• Experience in home and community care AND acute care


• Excellent written and verbal communication skills


• Experience with EPIC will be considered an asset 


• Ensure patient and family centric care


• Strong organization skills and able to work in a fast-paced environment


• Ensure frequent, effective, and timely communication between staff and external stakeholders (patients, families, referral sources, funders)


• Be a great team player and work in highly collaborative and interdisciplinary team


• Provide input into business development and/or service delivery model development opportunities


• Possess a valid driver’s license with access to a reliable and insured vehicle

Job Rotations:  12pm to 8pm on a rotating 2 week scheduled inclusive of weekends as below:

Week 1: Monday, Tuesday, Wednesday, Friday, Saturday and Sunday 12pm to 8pm (Thursday off) 

Week 2: Monday, Tuesday, Thursday, and Friday 12pm to 8pm (Wednesday, Saturday and Sunday off)
