Patient Access Manager at Fraser Health

As the Manager of Patient Access & Community Transition, you'll oversee patient flow, discharge planning, and coordination between hospitals and community care. Alongside clinical teams, your mission will be to identify and address barriers affecting patient transitions and facilitate seamless care. Your leadership will shape system-wide improvements in patient experiences and outcomes.

Key Responsibilities: • Manage patient access and transitions between healthcare settings • Collaborate on frameworks for efficient patient placement and flow • Monitor access and flow performance against set criteria • Create protocols for managing unique patient access demands • Lead complex discharge processes and resource management

Requirements: • Undergraduate degree in health sciences or equivalent • Seven years of experience in patient flow and discharge planning • Two years in a leadership position • Strong leadership, analytical, and problem-solving skills • Ability to develop effective working relationships
