Position Summary

We are seeking a dedicated and culturally grounded Case Coordinator to support our Prevention Program. The Case Coordinator will provide strengths-based, client-centered case management services to individuals, families, and community members, focusing on prevention, early intervention, and holistic wellness.

This role requires a strong understanding of Anisininew culture, traditions, and the impacts of intergenerational trauma. The successful candidate will work collaboratively with clients, partners, and interdisciplinary teams to promoting safety, and well-being.

Qualifications

• Must be a Registered Social Worker (RSW) in good standing or actively in the process of registration with the appropriate regulatory body
  
  • Underfill Opportunity: If you do not currently hold a BSW, but have completed a three-year social science degree or two-year college diploma viewed as desirable
• Bachelor of Social Work (BSW) required
• Experience in case management, prevention programs, or community social services
• Experience working with Indigenous communities strongly preferred

Key Responsibilities

• Provide comprehensive case management, including assessment, action planning, implementation and follow-up services
• Support individuals and families in accessing appropriate community resources and services
• Develop culturally relevant prevention and intervention plans
• Build trusting, respectful relationships with clients using a trauma-informed and culturally safe approach
• Collaborate with community agencies, Elders, Knowledge Keepers, and service providers
• Maintain accurate and timely case notes. Reports, and documentation
• Participate in program development, evaluation, and continuous improvement
• Advocate for clients and support self-determination and empowerment
• Ensure confidentiality and compliance with organizational policies and ethical standards
• Demonstrated flexibility when it comes to duties and responsibilities

Required Skills & Knowledge

• Strong understanding of Indigenous cultures, traditions, and community dynamics
• Knowledge of the impacts of colonization, residential schools, and intergenerational trauma
• Experience with trauma-informed, strengths-based, and culturally safe practice
• Excellent communication, interpersonal, and organizational skills
• Ability to work independently as well as part of a team
• Strong crisis intervention and problem-solving skills
• Proficiency in documentation and case management systems

Additional Requirements

• Valid Manitoba Class 5 driver’s license and access to a reliable vehicle
• Clear criminal record check and child abuse registry check
• Non-Violent Crisis Intervention Certification
• Willingness to travel within the community to conduct home visits/wellness checks
