The Clinical Pharmacist plays a crucial role in ensuring safe and effective medication use through comprehensive review, patient counseling, and collaboration with healthcare teams. This position requires expertise in drug interaction analysis, dosage adjustments, and clinical documentation to optimize patient outcomes while maintaining adherence to policies and safety standards.
Responsibilities
- Conduct thorough medication reviews to ensure appropriateness and safety
- Provide patient counseling on medication use and effects
- Adjust dosages based on clinical assessment and patient-specific factors
- Analyze drug interactions and monitor for adverse events
- Maintain accurate clinical documentation and reporting
- Collaborate with healthcare providers to optimize treatment plans
- Report and manage adverse drug reactions
- Ensure compliance with pharmacy policies and regulatory standards
- Perform sterile and non-sterile compounding and verification
Required Qualifications
- Minimum 3 years of experience in clinical pharmacy
- Bachelor's degree in Pharmacy
Preferred Qualifications
- Licensed Pharmacist
- Experience in Medication Therapy Management
- Proficient in patient counseling and interprofessional communication
- Strong attention to detail and problem-solving skills
- Effective time management abilities
- Experience with home infusion and sterile compounding
