Position # : 9003584 Expected Start Date: June 23, 2025 Union: HSAS Facility: Victoria Square City/Town: Prince Albert Department: Mental Health Community Case Management Type: Full-time temporary Expected Up to Date: June 29, 2026 FTE: 1 Hours of Work: 112 hours per 3 weeks Relief: No Float: No Field Hours: Yes Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: No : Reporting to the Manager of Mental Health and Addiction Services, the incumbent is responsible for the screening and coordination of initial service requests for all programs within Mental Health and Addictions Services in the former health region. The Intake Facilitator provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Intake Facilitator provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services. Human Resources Exemption: No Experience

• Bachelor of Social Work (BSW)

Licenses

• Valid Class 5 driver's license

Other Education and Training

• Bachelor of Social Sciences is acceptable
• Bachelor of Nursing is acceptable
• Bachelor of Psychiatric Nursing is acceptable
• Bachelor of Addictions Counseling is acceptable
• Psychiatric Nursing diploma is acceptable
