Our goal is to improve clinical outcomes and provide personalized care every step of the way. CareRx is looking for a full time Clinical Consultant Pharmacist to join our Calgary team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long‑term care, supportive living, and other congregate living communities. The role involves completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services. Role Accountabilities Act as liaison between long‑term care homes and retirement communities and CareRx Pharmacy to ensure optimal resident care, pharmacy services, and clinical support. Travel to LTC homes and retirement communities. Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams. Complete medication reviews in current retirement communities and long‑term care homes serviced by CareRx Pharmacy. Complete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulations. Provide education to residents and staff on health‑related topics, including the MedsCheck program, and attend community‑based wellness fairs and other events as required. Conduct education sessions within long‑term care homes and retirement communities within the scope of practice (e.g., policy and procedure, web portal, eMAR, influenza protocol). Support CareRx Pharmacy policies and procedures within retirement communities and long‑term care homes. Complete medication audits on site in LTC homes and retirement communities as required. Ensure regular on‑site visits to all assigned clients. Attend and actively participate in clinical team meetings. Participate in the development and implementation of industry‑leading clinical pharmacy service where applicable. Be an education, policy and pharmacy initiative resource. Be available for special clinical team projects as required. Collaborate on medication audit follow‑up improvement plans when applicable. Work collaboratively with other team members to support homes and communities. Participate in pharmacy‑related transition processes (e.g., new client transitions, eMAR transitions). Help with pharmacy operations/drug distribution when required. Qualifications Insured and currently licensed in good standing with the Alberta College of Pharmacy (ACP). Proven ability to foster strong partnerships with other members of the health care team. Effective organization, planning and time‑management skills. Knowledge of Kroll Windows application and Microsoft Office programs. Comprehensive understanding of provincial regulations governing LTC homes and retirement communities. Familiarity with provincial standards of practice for LTC pharmacies. Excellent customer service and interpersonal skills, oral and written communication skills, and ability to interact with people at varying levels of professional expertise. Strong critical thinking skills and ability to prioritize appropriately. Demonstrated initiative and creativity. Self‑directed with minimal supervision, capable of working independently. Motivated to achieve results reflecting a high standard of quality. Good listening skills and adaptability to changes in job environment. Dependable and responsible in execution of job duties. Primarily LTC homes and retirement communities in Calgary and surrounding area. Work Shifts
