The Occupational Therapist is part of an interdisciplinary team that provides consultation, direct care, capacity enhancement and support to older adults (and their partners in care), living in the community. The Occupational Therapist is expected to ensure the safety and security of the client(s) environment, to practice in accordance with the College of Occupational Therapists of Ontario standards, and the policies and procedures as defined by LOFT Community Services. The Occupational Therapist will be responsible for working with individuals from diverse backgrounds, contributing to the LOFT Program development, and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program . The standard Service Period for a H2H Program is sixteen (16) weeks in duration, with one Service Bundle to be assigned to the Patient and delivered over the course of this defined timeframe, with Services provided in accordance with the Patient Transitional Care Plan. The H2H program is designed to support adults 55+ facing complex mental health challenges, addiction, dementia and responsive behaviours along with physical health and care needs. This program will focus on individuals lacking access to appropriate supports while living in the community and other social determinants of health. The H2H LOFT Service Team will be working closely with CAMH partners as part of an integrated care model and ensures contact with referred individuals in the community within 24 hours. As part of this mobile team, staff will be travelling across the GTA to meet and work in a variety of environments to support individuals where they are at including in their homes. What You Will Do:
The Occupational Therapist is directly responsible for developing and articulating strategies for the management of recovery, psychosocial rehabilitation and coordination of care using information collected through assessments and collaboration with the integrated care team. Additionally, this role will liaise with community agencies, other practitioners and key partners, including caregivers, to coordinate care for clients.
- Meets the client's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining occupational therapy treatment plans in consultation with physicians or by prescription.
- Helps client develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving manual arts and crafts, practice in functional, prevocational, vocational, and homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities; directing aides, technicians, and assistants.
- Promotes maximum independence by selecting andconstructing therapies according to individual's physical capacity, intelligence level, and interest.
- Identify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour;
- Provide support with behaviours arising from cognitive issues, and contacting appropriate emergency services in crisis situations;
- Monitor client’s symptoms and initiate interventions to facilitate stabilization when required;
- Maintain all records of service and ensuring supplies and equipment are readily available;
- Conduct mandatory assessments and provide timely and accurate record keeping and reports as required;
- Assists with application of behavior management strategies under the guidance of the psychiatrist on the Hospital to Home team.
- Liaise with physicians, pharmacists and other health care providers regarding client care;
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required;
- Liaise, collaborate and coordinate with other professionals, service providers and partners within Hospital to Home program;
- Follows standard infection control practices and enhanced precautions as instructed;
- System/ resource navigation and case management coordinating care for patients as needed
- Liaise with community providers as needed to support implementation of clinical plan
- Develop short-term therapeutic relationships to foster coping and adaptation to health promoting strategies or chronic conditions.
- Promotes client and family focused care with a culturally diverse environment.
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services;
- Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required;
- Attend and actively participate in all required training;
- Collect data as required for statistical reporting purposes
- Participate in team building, program development and quality improvement activities
- Maintains safe and clean working environment by complying with procedures, rules, and regulations
- Making referrals to appropriate services.
- Provide ongoing client education
- Liaise, collaborate and coordinate with other professionals, service providers and partners within Integrated Psychogeriatric Outreach program;
- Other duties as required
- A Master’s degree in Occupational Therapy from an accredited institution;
- Must be registered and in good standing with the College of Occupational Therapists of Ontario
- Preferably ADP authorized
- An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
- Requires regular access to a vehicle or ability to travel/use public transit.
- Well-developed problem-solving abilities and interpersonal skills;
- Self-directed, results oriented and capable of working independently as well as in a team environment.
- Good oral and written communication skills and computer literacy.
- Excellent assessment skills, including demonstrated knowledge of crisis intervention skills.
- Demonstrated ability to provide individual/group health teaching to clients, caregivers and other care providers.
- Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members.
- Demonstrates awareness and understanding in working with individuals from different cultures.
- Demonstrates initiative and creativity in the workplace.
- Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members.
- Demonstrated strong ability to deliver services within professional scope of practice;
- Occasional evening and weekend hours may be required.
- Physically able to perform occasional client transfers in the home (as part of the assessment process).
- Extensive travel and use of own vehicle or transit is necessary.
