The Position
The Client Care Worker supports residents of supportive housing by coordinating care plans, collaborating with service providers, and guiding staff in delivering person-centered services. The role contributes to housing stability, wellness, and community connection by ensuring that residents have access to individualized supports, consistent follow-up, and opportunities to work toward personal goals.

Qualifications
- Diploma, preferably in Social Services.
- Minimum of 2 years’ experience supporting vulnerable populations with complex mental health and substance use needs.
- Knowledge of case management, trauma-informed practice, and the Residential Tenancy Act.
- Skills in conflict resolution, mediation, and counselling.
- Ability to support residents in skill development and goal attainment.
- Ability to collaborate with staff, clients, families, and community partners.
- Strong organizational, communication, and problem-solving skills
- Computer literacy and experience with client care information systems.
- Valid driver’s license and reliable vehicle.
- Vulnerable Sector and/or Criminal Record Check required.
Skills
- Strong case management and client support skills, including goal setting, care planning, and service coordination
- Ability to build trusting relationships and support residents in achieving housing stability and independence
- Knowledge of community resources, housing supports, and referral processes
- Experience supporting daily living skills, tenancy success, and basic financial management
- Ability to recognize health, safety, and wellness concerns and communicate effectively with clinical and support teams
- Knowledge of Trauma-Informed Practice, Harm Reduction, Housing First, Motivational Interviewing, and person-centered approaches
- Ability to provide culturally safe and inclusive services to diverse populations, including Indigenous residents
- Strong crisis intervention, de-escalation, and problem-solving skills
- Excellent documentation, record-keeping, and case management skills
- Strong communication and collaboration skills within multidisciplinary teams
- Ability to work independently, manage competing priorities, and participate in an on-call rotation
Working Condition and Physical Requirements
- Based in Lift’s Supportive Housing Client Care Office.
- Standard schedule of up to 35 hours per week and on-call rotation as needed.
- May involve transporting program supplies or resident belongings.
- Must be able to navigate stairs and perform light physical duties related to program support.
