Clinical Pharmacist in Community Settings

Join the mission to improve community health as a Clinical Pharmacist. Work collaboratively with healthcare teams to resolve medication issues and educate clients in various settings. You will support patient care under the guidance of the Pharmacy Clinical Programs Manager, focusing on seniors, end-of-life, and home care initiatives. Your role includes identifying drug therapy problems, conducting client visits, and delivering medication counseling. You will also be responsible for academic detailing to promote effective prescribing practices Key Responsibilities:

  • Identify and address drug therapy problems in community settings
  • Provide medication advice to clients and healthcare teams
  • Engage in home visits and clinic sessions for direct support
  • Deliver academic detailing to enhance prescribing standards
  • Build effective relationships with clients and care providers
Requirements:
  • License or eligibility for licensing in British Columbia
  • Completion of an Accredited Pharmacy Residency
  • Minimum 3 years of recent pharmacy experience
  • Experience in community practice highly desirable
  • Strong interpersonal and teaching abilities
