Clinical Program Consultant in Ontario

As a Program Consultant, you will provide clinical evaluations and treatment recommendations to improve patients' quality of life. Key Responsibilities:

• Conduct comprehensive clinical assessments for clients
• Develop individualized treatment plans for rehabilitation
• Engage in effective case management and support
• Collaborate with interdisciplinary teams on patient care
• Travel throughout North Simcoe Muskoka as needed

Requirements:

• Active registration with the Ontario College of Social Workers
• Undergraduate Degree in Social Work with relevant experience
• Valid Ontario driver’s license and a reliable vehicle
• Current experience in acquired brain injury social work
• Strong verbal and written communication skills
