Remote Addictions Counsellor - Intensive Treatment (Hospital Partnership)

About the role

We are seeking a compassionate and skilled Addictions Counsellor to join our multidisciplinary team. In this role you will serve within the Day/Evening Intensive Treatment program , which aims to provide intensive day treatment to individuals with substance use concerns. In partnership with Trillium Health Partners, William Osler Health Services and Punjabi Community Health Services, you will support clients post-discharge, providing case management and after-care support. This role involves processing referrals, group facilitation, therapeutic counselling, and case management, all while upholding the highest standards of care and professionalism.

This is a full-time, permanent role working at our head office on Dixie Road (Mississauga) and within the community as needed. Required to attend weekly meetings at the hospital and represent PAARC.

What you'll do

  • Facilitate structured content and process in-person and virtual psychoeducation groups, utilizing technology to improve access and care responsiveness.
  • Provide ongoing therapeutic counselling, case management, and client support.
  • Conduct comprehensive assessments, including risk and psychosocial assessments, and develop individualized care plans in collaboration with clients.
  • Act as a bridge between hospital in-patient services and PAARC’s community programs, ensuring clients experience a seamless transition.
  • Support clients in understanding their rights and treatment options, and advocate for their needs within the healthcare system.
  • Respond to crisis situations with appropriate interventions and support clients in developing and understanding healthy boundaries.
  • Initiate and maintain client files and documentation in accordance with best practices and legal guidelines, ensuring data accuracy and timeliness
  • Attend regularly scheduled meetings in a hospital setting including case consultation meetings and doing clinical rounds.

Requirements

  • Master’s degree in human services (completed or in progress) from an accredited institution.
  • Certification in Stage Screening and Assessment.
  • Experience in substance abuse counselling, with knowledge of withdrawal management and trauma-informed care.
  • Familiarity with anti-racism, anti-oppression frameworks, and a commitment to these principles.
  • Certifications in Non-Violent Crisis Intervention, CPR, and First Aid.
  • Strong communication skills, both verbal and written.
  • Ability to work effectively as part of a multidisciplinary team.
  • Transportation: Valid Ontario Driver’s License, insurance, and reliable vehicle required.
  • Language Skills: Proficiency in prevalent languages in the Region of Peel and French considered a strong asset.

Working Conditions

  • 35-hour work week; Mon - Fri 8:30 am- 4:30 pm.
  • Working primarily in-office
  • Travel within Peel Region to meet clients in various community based settings including a hospital setting
