MDS Back-up Coordinator in Ontario

Take on a vital role as an MDS/RAI Back-up at our Scarborough long-term care facility. This full-time position emphasizes coding accuracy and team empowerment. Reporting to the Director of Care, you will oversee RAI MDS processes and provide necessary training and support to the interdisciplinary team. This role requires a results-driven attitude, excellent communication skills, and an ability to analyze data effectively, all while enhancing resident care. Key Responsibilities:

• Achieve high scores on Relias assessments
• Lead and mentor the interdisciplinary team
• Schedule MDS assessments for all residents
• Review RAI submissions to minimize errors
• Analyze clinical data to improve care quality

Requirements:

• Current CNO registration as a Practical Nurse
• 3-5 years experience in Long Term Care
• Deep understanding of MDS/RUG III patient classification
• Knowledge of project management and quality improvement
• Effective communicator both orally and in writing
