Flexible Nurse Practitioner Role in Seniors Care

Make a difference as a Nurse Practitioner with Perennial, delivering integrated primary care to seniors in Ontario, Saskatchewan, Alberta, Manitoba, and Quebec. As a valued member of our interdisciplinary team, you'll provide comprehensive primary care services for senior patients. This role offers full-time or part-time options and empowers you to utilize your clinical expertise while collaborating with physicians and other healthcare professionals to ensure residents receive optimal care. Key Responsibilities:

Conduct routine check-ups and manage chronic conditions
Order and interpret diagnostic tests for seniors
Develop personalized care plans and prescribe medications
Address daily medical concerns from residents and families
Collaborate closely with a multidisciplinary healthcare team

Requirements:

Minimum of 2+ years nursing experience in relevant fields
Master of Nursing or equivalent graduate degree
Current NP certification and licensure in good standing
Strong assessment and development of care plans
Exceptional communication skills for patient education
