As a Nurse Practitioner at UHN, you will engage in autonomous patient care, addressing health disparities among marginalized populations. This casual role emphasizes advanced health assessment, care plan development, and interdisciplinary collaboration. You will work closely with community partners and support high-utilizing patients within the Emergency Department. Key Responsibilities:
- Manage a caseload of patients with complex needs
- Conduct comprehensive health assessments and diagnoses
- Develop and implement care plans using clinical data
- Collaborate with interdisciplinary teams for patient-centered care
- Mentor and lead members of the Social Medicine team
- Current registration in the Extended Class
- 4+ years of experience in mental health and primary care
- Strong focus on culturally competent care
- Ability to engage with diverse populations
- Valid driver's license for patient visits
