The Hamilton Niagara Haldimand Brant (HNHB) Organization of Palliative Care Physician Services/Enhancements (HOPE) is a clinical organization funded by the Ontario Ministry of Health and Long-Term Care (MOHLTC) Alternate Funding Plan (AFP). The Department of Family Medicine has over 240 staff and 1,500 faculty distributed throughout 8 teaching sites and surrounding communities, teaching 200 residents and over 650 other health professional learners annually, and serving over 1,000,000 patients within southern/south Central Ontario. It is a recognized leader in family medicine teaching, research and practice nationally and internationally.

Job Summary

The primary role of the HOPE Administrator (Manager) is to provide effective and efficient management of the Hamilton Niagara Haldimand Brant Organization of Palliative Care Physician Services/Enhancements (HOPE) organization. HOPE is a clinical organization funded by the Ontario Ministry of Health (MOH) Alternate Funding Plan (AFP). This shall include responsibility to develop and implement program growth, provide strategic leadership in system planning and lead and facilitate negotiations on behalf of HOPE with its numerous stakeholders and partners.

The Manager will be responsible for the leadership and day-to-day management of support staff while being accountable for the workload measures and deliverables set forth in the HOPE's AFP Service Delivery Plan of the MOH contract. The manager is accountable for the management and administration of the financial resources, sets and monitors the annual budget ensuring that adequate financial controls are in place. The HOPE Manager must perform duties in accordance with the policies and procedures of the Ontario Ministry of Health and Long-Term Care (MOHLTC) and McMaster University.

This position is guided by the core values, mission and vision of the HOPE organization and is responsible for monitoring the strategic plan to ensure alignment with the mission/vision of the organization. This will include ensuring a mature Governance Structure is in place and maintained. The Manager is also responsible for maintaining a HOPE profile within the HNHB.

The position requires both self-directed abilities, team, financial, negotiation and strong conflict resolution skills and as such demonstrates McMaster's six core leadership competencies; Communicating, Developing People, Leading, Embracing Change, Optimizing Service Relationships and Employing University Acumen.

Accountabilities

The HOPE Program Administrator (Manager) is accountable to the HOPE physicians, via the HOPE Executive (HOPE Lead, Associate Lead, and Past Lead) for all aspects of the HOPE program delivery of physician's services. The HOPE Program Manager is accountable to the Director of Administration, Department of Family Medicine, for aspects of the position pertaining to University employees, which may include finances, human resources and other aspects of the job performance.

Leads the HOPE Physician Group:

  • Organizes and attends all meetings of the HOPE Program.
  • Supports HOPE physicians to foster and support delivery of the MOHLTC contract for and for the group as whole.
  • Supports HOPE physicians ensuring that their clinical work is sustainable and host site.
  • Provides the HOPE physicians with appropriate information around required deliverables and supports the MD's in attaining these contractual deliverables.
  • Provides the HOPE Physicians with appropriate and relevant information regarding all aspects pertaining to the HOPE organization - budgets, funding, base rate increases, system planning issues etc.

Information Management:

  • Leads the development and maintenance of a database system for the HOPE program to fulfill the reporting requirements under the contract.
  • Manages, oversees and monitors the process of collection and producing all Shadow Billing requirements ensuring expedite submission to MOH via RMA .
  • Develops, implements and supervises a process of collecting relevant program information including all required workload measurement data and revenue distribution.
  • Manages the RMA contract for purchased services which include establishing, overseeing and maintaining a process for all record keeping and production of reports including all Group Service reports. Monthly Indirect Service reports, Quarterly patient reports, audited financial reports and any such reports and records that may be requested.
  • Manages an electronic medical record (EMR) system for the HOPE physicians working in the community. This includes providing access to learners on a monthly basis and ensuring a diligent process for disabling access.
  • Develops, implements and maintains a process for outcomes evaluation and quality management .
  • Composes and edits all correspondence of the HOPE Program and its relationships with the broader Palliative Care system including the HNHB, the Province of Ontario and beyond.

Financial Management and Administration:

  • Develops and manages a budget of approximately $4.5 million for all services and programs of the HOPE program.
  • Develops and manages the HOPE administrative budget and works with the HOPE Leads to determine priorities.
  • Maintains financial records and works with RMA and the Department of Family Medicine to maintain books for accounting regarding the receipt and administration of all funds of the HOPE Program for seven years.
  • Oversees a program of transaction records and retention of such records long for a minimum of seven years.
  • Manages, monitors and maintains contractual obligations with the Ontario MOHLTC to ensure a successful funding and program relationship.

Human Resource Management and Administration:

  • Oversees the process for physician recruitment - posting , along with the HOPE Lead meet with each candidate to review AFP, organize interviews, and complete reference checks.
  • Manages and oversees process for new physicians to complete necessary AFP MOH contract papers as well as HOPE's paperwork (Governance Agreement and policies).
  • Analyzes human resource requirements for the HOPE Program within the context of both administering the program and of changing program needs and growth.
  • Coaches, guides, supports, monitors and supervises staff when applicable.
  • Works with the HOPE Executive to write position descriptions and oversees the hiring of administrative staff.
  • Revisits and revises role descriptions, roles and responsibilities of individual staff within the context of changing program needs and requirements.
  • Conducts performance appraisals on an annual basis or more frequently as determined/needed.
  • Disciplines and/or terminates staff as required (in collaboration with the Department of Family Medicine).
  • Oversees the day-to-day work of staff, including attendance, performance, lieu time, conflict identification and resolution.
  • Plans and implements staff training, team building and strategic planning.

Internal Governance:

  • Works with the HOPE Executive to develop and create HOPE's Mission, its Core Values, and Vision based on the group's shared values.
  • Works with the HOPE Executive to establish HOPE's Strategic Plan based on its mission and vision and the current climate.
  • Monitors HOPE's Strategic Plan to ensure it is aligned with the current climate, fostering change as needed.
  • Works with HOPE Executive to lead the program through periods of planned growth, evolution and change.
  • Works with the HOPE Executive to develop/revise a robust, mature Governance Structure and policies.
  • Monitors Governance Agreement and Policies and identifies changes as needed.
  • Leads in the development and maintenance of a Team Model for the Program.
  • Meets regularly with partners involved in the purchase of service agreements for the purpose of planning, reporting, information sharing, role clarification and issues resolution etc.

Stakeholder Relationship Management - External:

  • Works with the HOPE executive in the strategic planning and positioning of the HOPE Organization as a leader in delivery of a justice- based system of Palliative Care within the HNHB.
  • Develops and fosters supportive and positive relationships with a range of stakeholders such as, but not limited to; hospitals, HNHB End of Life Care Network, allied health professionals, HNHB Home and Community Care, government, university and other groups as required.
  • Liaises with the Department of Family Medicine, and the Division of Palliative Care and the various academic programs associated therein to support their academic missions.

Communications Management:

  • Chairs HOPE meetings and other meetings with stakeholders.
  • Works with the HOPE executive to negotiate with stakeholders regarding HOPE physician services.
  • Facilitates and fosters a “culture of teamwork” with HOPE members.
  • Manages and resolves conflict when appropriate.
  • Communicates HOPE's identity, mission, purpose and vision.

Qualifications

Education:

  • Bachelor's degree in Business Administration, or related field with a focus on health care or health care administration, or equivalent.

Experience:

  • 3-5 years of relevant administrative management and supervisory experience
  • Experience in program development, delivery and evaluation
  • Experience working with government agencies and representatives
  • Experience in a health care/medical environment essential

Knowledge/Skills:

  • Knowledge of Palliative Care programs especially in terms of local, regional and Ontario systems planning levels in an academic environment
  • Ability to work effectively with a physician group and interprofessional team environments
  • Ability to be self-starting and self-directing
  • Ability to attend to detail and accuracy
  • Strong administrative, organization and leadership skills
  • Strong communication and interpersonal skills
  • Computer skills involving a wide range of programs ranging from data entry to presentation preparation
  • Knowledge of emerging information technology systems in health care including patient data and workload software systems
