Join Acclaim Health as a compassionate Program Coordinator for the Adult Day Program. This full-time position involves working Monday to Friday, 11:30 am to 8:00 pm, fostering an engaging environment for participants.
In this role, you will coordinate daily programming, support participants with diverse needs, and ensure a safe atmosphere. Strong communication skills are essential as you balance administrative tasks and hands-on support. Collaboration with staff and families is key, ensuring program adaptations to meet individual goals and client-centered service plans.
Key Responsibilities:
• Coordinate client check-in/check-out for respite services
• Monitor client status and updating care plans
• Lead staff during emergencies and responsive behaviors
• Participate in on-call rotation for supervisory support
• Assist clients with activities of daily living as needed
Requirements:
• Community College diploma in Gerontology or Social Services
• 2-3 years experience with adults with special needs
• CPR certification required
• Proficient in Microsoft Office; AlayaCare is a plus
• Valid Ontario driver's license and insurance required
